ABSTRACT

OBJECTIVE@#To investigate the effect of modified LMB 89± rituximab regimen on long-term clinical benefit for patients with Burkitt lymphoma.@*METHODS@#Clinical data of 43 patients with Burkitt lymphoma were collected and retrospectively analyzed in the period from July 2006 to October 2017. Forty-three patients were divided into 2 groups, including Hyper-CVAD regimen, R-EPOCD regimen or VDCLP regimen treated group as control (20 patients) and modified regimen group with modified LMB 89±rituximab regimen (23 patients); the event-free survival (EFS) rate, overall survival (OS) rate, cumulative incidence of relapse (CIR), non-relapse mortality rate (NRM) and adverse reaction incidence of 2 groups were compared. At the same time, the efficacy analysis for patients of modified regimen group was performed according to age and using rituximab or no.@*RESULTS@#The EFS rate and OS rate of modified regimen group were significantly higher than those of control group (P<0.05). The CRR rate and NRM rate of modified regimen group were significantly lower than those of control group (P<0.05). The EFS rate and OS rate of patients for <40 years old were significantly higher than those of patients for ≥40 years old in modified regimen group (P<0.05). The EFS rate and OS rate of patients with rituximab were significantly higher than those of patients without rituximab in modified regimen group (P<0.05). There was no significant difference in the adverse reaction incidence between 2 groups (P>0.05).@*CONCLUSION@#Modified LMB 89± rituximab regimen in the treatment of patients with Burkitt lymphoma can efficiently prolong survival time and shows the better safety; and the remigen combined with rituximab is more helpful to improve the clinical prognosis and show the better clinical effects for the patients≤40 years old.

ABSTRACT

OBJECTIVE@#To investigate the curative efficacy of first generation TKI in the treatment of CML-CP combined with vPh and genetic characteristics.@*METHODS@#60 patients with CML-CP combined with vPh from January 2010 to May 2017 in the First Affiliated Hospital of Kunming Medical University were chosen as CML-CP-vPh group, and 107 patients with CML-CP combined with typical Ph chromosome at the same time were chosen as control group. The patients in two groups were treated with imatinib; The curative efficacy, karyotype and FISH signal type were compared between 2 groups, and the factors influencing long-term survival of patients were analyzed by Cox risk model.@*RESULTS@#There was no significant difference in the demographic and hematological baseline data between 2 groups (P>0.05), and there was no significant difference in the incidence of drug-resistance between 2 groups (P>0.05). The incidence of primary drug-resistance and primary hematological drug-resistance in the CML-CP-vPh group were significantly higher than that in control group (P<0.05). There was no significant difference in the accumulative CCyR rate, accumulative MMR rate, OS and EFS between 2 groups (P>0.05). Multivariate Cox model analysis showed that vPh not correlated with OS and EFS of patients with CML-CP (P>0.05). The factors influencing OS in CML-vPh patients included high risk of Sokal scores, peripheral blood basophils proportion ≥10%, BCR-ABL in 3 months after treatment<10%, achieviag CCyR in 6 months after treatment and achieviag MMR in 12 months after treatment (P<0.05). The factors influencing EFS included BCR-ABL<10% in 3 months after treatment and achieving MMR in 12 months after treatment (P<0.05). The regions with high frequency of heterotopic involvement included 12q1, 12q2 [9 cases (15.00%)] and 1p3 [8 cases (13.33%)]. The percentage of 2G2R1Y, 1G1R2F, 1G2R1Y, 2G1R1Y and 1G1R1Y in FISH signal types were 73.33%, 10.00%, 1.67%, 1.67% and 1.67% respectively.@*CONCLUSION@#Patients with CML-CP combined with vPh possess higher primary drug-resistance rate and primary hematological drug-resistance rate for the first generation TKI, while second-generation TKI can efficiently improve long-term survival, its efficacy is similar to efficacy for patients with typical Ph chromosomes. CML-CP combined with vPh does not display special demographic and hematological characteristics. The main involved regions of heterotopic variants include 12q1, 12q2 and 1p3, while 2G2R1Y type is the most common type of FISH signal.

ABSTRACT

This study was designed to investigate the inhibitory effect and mechanism of neferine (Nef) on invasion and metastasis of nasopharyngeal carcinoma cells (NPC). The viability of CNE-1 and 5-8F cells was detected by CCK-8 assay after treatment with different concentrations of Nef. The effects of Nef on cell migration and invasion were detected by the scratch test and Transwell assay. Western blot analysis was used to detect the effects of Nef on levels of epithelial-mesenchymal transition (EMT)-associated proteins and transcription factors. The differentially expressed gene profiles between control group and Nef group were analyzed by microRNA microarray, combined with bioinformation analysis. It was observed that 30 μmol·L-1 Nef had no significant effect on the viability of CNE-1 and 5-8F cells. Western blot assay showed that the expression level of neurotroponin cadherin (N-cadherin) and vimentin decreased after treatment with Nef, while the expression of epithelial cadherin (E-cadherin) increased. The expression of transcription factors including Twist, Snail, and Slug exhibited no significant difference. Results of the microRNA microarray suggest that 10 microRNAs showed significant differences when compared with the control group. Bioinformatics analysis showed that hsa-let-7c-5p and hsa-microRNA-423-5p targeted the same downstream genes: small integral membrane protein 3 (SMIM3) and nerve growth factor (NGF). Overexpression of hsa-let-7c-5p and hsa-miR-423-5p promoted the invasion and migration ability of 5-8F cells and decreased the expression of SMIM3 and NGF. The results from this study suggest that Nef may inhibit the invasion and metastasis of NPC cells by inhibiting the expression of hsa-let-7c-5p and hsa-miR-423-5p followed by the upregulation of SMIM3 and NGF; thus, regulating the expression of EMT-associated proteins. Our data have provided experimental evidence for the inhibition of tumor invasion and metastasis by Nef.

ABSTRACT

INTRODUCTION@#Family history of psychopathology is a risk factor for mood and anxiety disorders in children, but little is known about rates of parental psychopathology among treatment-seeking youth with affective disorders in the Asia Pacific region. This study examined patterns of emotional and behavioural problems in parents of clinically-referred youth in Singapore. We hypothesised that parents would have higher rates of affective disorders compared to the Singapore national prevalence rate of 12%.@*MATERIALS AND METHODS@#In this cross-sectional study, 47 families were recruited from affective disorders and community-based psychiatry programmes run by a tertiary child psychiatry clinic. All children had a confirmed primary clinical diagnosis of depression or an anxiety disorder. Parents completed the Mini International Neuropsychiatric Interview (MINI) to assess for lifetime mood and anxiety disorders. They also completed the Adult Self Report (ASR) and Adult Behavior Checklist (ABCL) to assess current internalising and externalising symptoms.@*RESULTS@#Consistent with our hypothesis, 38.5% of mothers and 10.5% of fathers reported a lifetime mood and anxiety disorder. Nearly 1/3 of mothers had clinical/subclinical scores on current internalising and externalising problems. A similar pattern was found for internalising problems among fathers, with a slightly lower rate of clinical/subclinical externalising problems.@*CONCLUSION@#Our findings are consistent with previous overseas studies showing elevated rates of affective disorders among parents - particularly mothers - of children seeking outpatient psychiatric care. Routine screening in this population may help to close the current treatment gap for adults with mood and anxiety disorders.

ABSTRACT

@#AIM: To evaluate the clinical outcomes in terms of vision across distances(near, intermediate and far), contrast sensitivity and subjective patient satisfaction after femtosecond laser-assisted cataract surgery(FLACS)with implantation of an extended range of vision(ERV)intraocular lens(IOL).<p>METHODS: Forty patients(55 eyes)underwent bilateral or monocular FLACS with implantation of the ERV IOL Tecnis Symfony(Johnson & Johnson Vision)were enrolled. Uncorrected distance(UDVA), intermediate(UIVA)and near visual acuities(UNVA)were evaluated at 3mo after surgery, as well the defocus curve, contrast sensitivity, patient satisfaction and spectacle independence.<p>RESULTS:No severe complications occurred. All eyes showed a central position of the IOL in the capsular bag without tilting at 3mo after surgery. 3mo postoperative mean logMAR visual acuity at 5 m, 67 cm and 40 cm were -0.04 ±0.08, -0.17±0.22, 0.37±0.17, respectively. All patients obtained satisfactory UDVA and UIVA, as well as functional UNVA, meeting the needs of daily life. Spectacle independence rate was 94.55%. Contrast sensitivity results did not differ from those obtained with monofocal aspheric lenses. Likewise, no moderate and severe photic phenomena were reported. Mean patient satisfaction scores with distance, intermediate and near vision were 9.0, 9.0, and 7.0, respectively.<p>CONCLUSION: FLACS with implantation of the ERV IOL TECNIS Symfony provides a successful visual restoration at far, intermediate distance and a functional-range near vision acuity, with minimal level of disturbing photic phenomena, and high rates of spectacle independence and patient satisfaction.

ABSTRACT

<p><b>OBJECTIVE</b>To study the value of transesophageal atrial pacing (TEAP) in neonates with tachyarrhythmia.</p><p><b>METHODS</b>The clinical data of 26 neonates with tachyarrhythmia who underwent TEAP electrophysiological examination or cardioversion were collected.</p><p><b>RESULTS</b>Of the 26 neonates, 15(58%) were diagnosed with atrioventricular reentrant tachycardia, 3(12%) were diagnosed with sinus tachycardia, 3(12%) were diagnosed with ventricular tachycardia, 2(8%) were diagnosed with fast/slow atrioventricular nodal reentrant tachycardia, 2(8%) were diagnosed with atrial tachycardia, and 1(4%) was diagnosed with sinus tachycardia with ventricular preexcitation. Overdrive suppression was performed for 22 neonates, among whom 18 achieved successful cardioversion, and 2 with atrial tachycardia and 2 with ventricular tachycardia failed to restore sinus rhythm.</p><p><b>CONCLUSIONS</b>TEAP is helpful to the diagnosis of tachyarrhythmia in neonates and can bring about a high rate of cardioversion success.</p>

ABSTRACT

Objective To investigate the effect of TGF-β1 on the proliferation of lymphoma cells induced by M2 macrophages. Methods TGF-β1 was used to treat Jiyoye lymphoma cells, and MTT was used to determine the cell proliferation. Lymphoma cells were treated with M2 macrophage culture liquid supernatant and TGF-β1. MTT assay was used to assess the lymphoma cell proliferation, Transwell assay was used to assess the cell migration and invasion, and Western blot was perfomed to determine the levels of MMP-2, MMP-9, PCNA and Ki-67 proteins. Results The lymphoma cells after TGF-α treatment showed that the proliferation ability, the number of invaded cells and the number of migrated cells, and the levels of MMP-2, MMP-9, PCNA and Ki-67 were significantly decreased, compared with the untreated lymphoma cells (P< 0. 05). After treated with the supernatant of M2 type macrophage culture supernatant, the proliferation ability of lymphoma cells, the number of invaded cells and migrated cells, the levels of MMP-2, MMP-9, PCNA and Ki-67 proteins were significantly increased, compared with the untreated lymphoma cells (P < 0. 05 ). Compared with the lymphoma cells treated with culture medium supernatant of M2 macrophages alone, the lymphoma cells treated with TGF-β1 and M2 macrophage culture medium supernatant showed that the proliferation ability, number of invaded and migrated cells, and the protein levels of MMP-2, MMP-9, PCNA and Ki-67 were significantly increased (P<0. 05). Conclusions TGF-β1 can reduce the proliferation, invasion and migration of lymphoma cells induced by macrophages. The mechanism of action is related to the expression of MMP-2, MMP-9, PCNA and Ki-67 proteins.

ABSTRACT

Objective To systematically evaluate risk factors for healthcare-associated infection(HAI)in patients with acute myocardial infarction(AMI).Methods Literatures about risk factors for HAI in AMI patients were re-trieved from PubMed,EMbase,the Cochrane Library,China Biological Medicine Disk (CBM),WanFang Data, China National Knowledge Infrastructure (CNKI),and VIP Database from the establishment of database to Decem-ber 2017,two independent researchers screened literatures,extracted data,and evaluated quality according to inclu-sion and exclusion criteria,then performed Meta analysis using RevMan 5.3 software.Results There are 12 litera-tures in accordance with the inclusion criteria,with a total sample size of 13 812 cases.According to Meta analysis, 12 risk factors were statistically significant (all P<0.05):age (OR ,3.45 [95%CI ,1.02-7.95]),length of hos-pital stay(OR ,4.84 [95%C I ,2.47-7.14]),tracheal intubation (OR ,7.74 [95%CI ,3.73-9.02]),preventive use of antimicrobial agents (OR ,6.79 [95%CI ,2.38-9.45]),mechanical ventilation (OR ,2.98 [95%CI ,1.05-4.78]),urinary catheterization (OR ,3.56 [95%CI ,1.24-4.49]),heart failure(OR ,3.82 [95% CI ,2.53-5.74]),arrhythmia (OR ,2.47 [95%CI ,1.28-4.63]),diabetes (OR ,2.23 [95% CI ,1.62-3.27]),chronic obstructive pulmonary disease (OR,2.14 [95%CI ,1.83-3.91]),use of H2blockers (OR,2.61 [95%CI ,1.87-4.36]),and left ventricular ejection fraction<30%(OR,2.98 [95%CI ,1.35-4.26]).The publication bias of each risk factor was analyzed,and the results showed that publication bias was not obvious.Conclusion Health care workers should timely identify the high-risk population for HAI,strictly adhere to indications of invasive procedure, standardize the disinfection and isolation system,strengthen the process management of aseptic technique,and ac-tively take preventive measures to reduce risk for HAI in AMI patients.

ABSTRACT

Objective To provide advice and basis for further reform of the medical service price by analyzing the effect on hospital expenses after calling off drug-addition and price adjustment of medical service in public hospitals at the end of 2016. Methods Comparing the changes of hospital expenses based on the data of hospital expenses before and after the implementation of the price reform policy in a provincial public hospital in Sichuan. Results After canceling the drug-addition and adjustment of medical service price, whether in the whole hospital, in the surgical department or in the non-operative department, the average cost of drugs for discharged patients has been reduced, the average costs of surgical operation and anesthesia has been increased, the ratio of change remains consistent with the policy adjustment range, the composition of drug charges and bed fee has been decreased and the proportion of drugs still ranks first in the composition ratio. Conclusion Canceling the drug-addition and price adjustment of medical service have a significant effect on hospital expenses. The adjustment of medical service price reflects the technical value of medical staff.

ABSTRACT

Objective To investigate the influencing factors of portal vein tumor thrombus in patients with primary liver cancer(PLC).Methods A total of 483 patients with PLC in the Second People's Hospital of Fuyang from January 2012 to December 2016 were included in this retrospective study.Univariate analysis and multi-variate logistic regression analysis were employed to analyze the association between portal vein tumor thrombus and routine clinical parameters.Results The univariate analysis showed that alanine aminotransferase,aspartate aminotransferase,total bilirubin,γ-glutamyltranspeptidase,alkaline phosphatase,alpha-fetoprotein,the size and the number of tumors,ascites and center location of cancer performed a close relationship with the formation of portal vein tumor thrombus in patients with PLC,and the differences were statistically significant(P values were 0.010,0.000,0.010,0.000,0.001,0.000,0.000,0.004 and 0.010).Multivariate logistic regression analysis showed that potential significant predictors of portal vein tumor thrombus in patients with PLC were alpha-fetoprotein and ascites,and the differences were statistically significant(P values were 0.000 and 0.040).Conclusion PLC patients with high level of alpha-fetoprotein and ascites are easier to induce the formation of portal vein tumor thrombus.

<p><b>OBJECTIVE</b>To investigate the neurodevelopmental level of preterm infants at the corrected age of 1 year and the effect of complications on neurodevelopment.</p><p><b>METHODS</b>The clinical data and follow-up data of hospitalized preterm infants were retrospectively studied. The Bayley Scales of Infant Development was used to assess the neurodevelopmental level. Preterm infants were divided into groups according to gestational age, birth weight, and the presence or absence of complications. The mental development index (MDI) and psychomotor development index (PDI) were compared between groups.</p><p><b>RESULTS</b>At the corrected age of 1 year, compared with the late preterm infants, the early preterm infants had significantly lower MDI and PDI (P<0.05) and significantly higher rates of retarded intellectual and psychomotor development (P<0.01). Compared with the normal birth weight group, the low birth weight group had significantly lower MDI and PDI (P<0.01) and significantly higher rates of retarded intellectual and psychomotor development (P<0.01). The preterm infants with hyperbilirubinemia, birth asphyxia or neonatal respiratory distress syndrome (NRDS) had significantly lower MDI and PDI than those without such complications (P<0.05).</p><p><b>CONCLUSIONS</b>Lower gestational age and birth weight are associated with worse intellectual and psychomotor development in preterm infants. Complications, such as hyperbilirubinemia, birth asphyxia and NRDS, have adverse effects on neurodevelopment of preterm infants.</p>

ABSTRACT

<p><b>INTRODUCTION</b>This study aimed to investigate the risk factors associated with mortality in haematopoietic stem cell transplant (HSCT) patients admitted to our paediatric intensive care unit (PICU) over an 8-year period.</p><p><b>MATERIALS AND METHODS</b>A retrospective chart review was conducted of all HSCT patients requiring PICU admission at our centre (a tertiary care university hospital in Singapore) from January 2002 to December 2010. Chief outcome measures were survival at the time of PICU discharge and survival at 6 months after initial PICU admission.</p><p><b>RESULTS</b>Ninety-eight patients underwent HSCT during this period; 18 patients (18%) required 24 PICU admissions post-HSCT. The overall survival to PICU discharge was 62.5%. Of those who survived discharge from the PICU, 33% died within 6 months of discharge. Non-survivors to PICU discharge had a higher incidence of sepsis (89% vs 33%,= 0.013) and organ failure as compared to survivors (cardiovascular failure 100% vs 20%,= 0.0003; respiratory failure 89% vs 20%,= 0.002; and renal failure 44% vs 7%,= 0.047). Mortality rates were higher in patients requiring mechanical ventilation (70% vs 14%,= 0.010) and inotropic support (70% vs 14%,= 0.010). Mortality in all patients with renal failure requiring haemodialysis (n = 4) was 100%. Presence of 3 or more organ failures was associated with 80% mortality (= 0.003).</p><p><b>CONCLUSION</b>Sepsis, multiple organ failure and the need for mechanical ventilation, inotropes and especially haemodialysis were associated with increased risk of mortality in our cohort of HSCT patients.</p>

ABSTRACT

: One of the most feared complications of childhood cancer treatment is second malignant neoplasms (SMNs). This study evaluates the incidence, risk factors and outcomes of SMNs in a tertiary paediatric oncology centre in Singapore.: A retrospective review was conducted on patients diagnosed with childhood cancer under age 21 and treated at the National University Hospital, Singapore, from January 1990 to 15 April 2012. Case records of patients with SMNs were reviewed.: We identified 1124 cases of childhood cancers with a median follow-up of 3.49 (0 to 24.06) years. The most common primary malignancies were leukaemia (47.1%), central nervous system tumours (11.7%) and lymphoma (9.8%). Fifteen cases developed SMNs, most commonly acute myeloid leukaemia/myelodysplastic syndrome (n = 7). Median interval between the first and second malignancy was 3.41 (0.24 to 18.30) years. Overall 20-year cumulative incidence of SMNs was 5.3% (95% CI, 0.2% to 10.4%). The 15-year cumulative incidence of SMNs following acute lymphoblastic leukaemia was 4.4% (95% CI, 0% to 8.9%), significantly lower than the risk after osteosarcoma of 14.2% (95% CI, 0.7% to 27.7%) within 5 years (<0.0005). Overall 5-year survival for SMNs was lower than that of primary malignancies.: This study identified factors explaining the epidemiology of SMNs described, and found topoisomerase II inhibitor use to be a likely risk factor in our cohort. Modifications have already been made to our existing therapeutic protocols in osteosarcoma treatment. We also recognised the importance of other risk management strategies, including regular long-term surveillance and early intervention for detected SMNs, to improve outcomes of high risk patients.

Objective To investigate the trend of early neural development in premature infants. Methods At the age of 12 months and 24 months, Bayley Scales of Infant Development were used to assess the mental development index (MDI) and the psychomotor development index (PDI) in preterm (corrected age) and full-term infants. Results At 12 months, there was no significant difference in corrected age PDI scores among different gestational age groups (<32 , 32–33+6 and 34–36+6 weeks) (P=0.820). The actual age MDI and PDI scores of full-term infants and premature infants in 34~36+6 weeks group were significantly higher than those of premature infants in <32 and 32-33+6 weeks groups, and the PDI score of full-term infants was significantly higher than that of premature infants in 34-36+6 weeks group (P<0.05). There was no significant difference in actual age PDI scores among different birth weight groups (P=0.166). The actual age MDI and PDI of full-term infants and premature infants in birth weight≥2500 g group were significantly higher than those of premature infants in <1500 g, 1500~1999 g and 2000~2499 g groups (P<0.05). At 24 months, the actual age MDI scores of full-term infants were significantly higher than those of premature infants in different gestational age and birth weight groups (P<0.05). The actual age MDI curve of premature infants in birth weight <1500g group showed a downward trend, while the actual age PDI curve showed a significant upward trend. Conclusion The neurodevelopment of preterm infants at the corrected age of 12 and 24 months reaches the level of full-term infants.

Many voting activities such asThe Best Medical Workers put medical workers onto moral high ground,while medical dispute,medical anomie and other news pushed them to the abyss of criticism.These are all involved in the pan-moralization of medicine-related consensus of mass media.Taking specific opinion as example,this paper listed the pan-moralization characteristics including judging the complicated problems with ethical standard merely,evaluating behaviors of physicians and patients with idealistic principles,determining the whole moral outlook from the individuals,analyzed the reason of social emotional outbursts,traditional moral interference,media ethics anomie,and lack of public rationality,and finally proposes the strategies to avoid pan-moralization of medicine-related consensus from the micro and macro level.

ABSTRACT

Objective To construct EHEC△z5150 using Red recombination and explore its pathogenicity.Methods Overexpression of the z5150 gene was used to test its toxicity.The kanamycin resistant gene flanked by homologues of target genes was amplified by PCR before being transferred into EHEC containing pKD 46 plasmid,and the target fragments were recombined into genome by Arabia sugar.The kanamycin resistance gene was removed by pFLP 2 plasmid mediated FLP site-specific recombination.Cell adhesion ability and virulence of the mutant were detected by infecting HT 29 cells and BALB/c female mice,respectively.Results and Conclusion The mutant strain EHEC △z5150 was successfully constructed.The absence of z5150 did not affect the normal growth of bacteria,but after excessive expression,it inhibited the growth.Deletion of z5150 showed significant difference in intestinal epithelial cell adhesion and pathogenicity compared with wild strain.This study provides a gene knockout technique for the study of EHEC gene function and of the relationships between RelEz5150 and EHEC pathogenicity.

According to the professional characteristics of the operating room,on the basis of basic principles of hierarchical management and post management of nursing department,taking clinical needs as guiding ideology,combined with the development needs of surgical departments,we developed the operating room nursing staff management model,and set up 23 jobs.We adjusted it accordingly in practice,and constantly optimized nurses' career plan by providing clinical,teaching,research,management and other multi-channel development direction,meanwhile provided better quality care for patients.

AIM To study the chemical constituents from the twigs of Morus alba L..METHODS The 60% ethanol extract from M.alba twigs was isolated and purified by ODS,sephadax LH-20 and semi-preparative column,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Ten compounds were isolated and identified as 4'-O-β-D-glucopyranosyl-2'-hydroxyresveratrol 3-(6-O-β-D-glucopyanosyl-β-D-glucopyranoside) (1),3-O-β-D-glucopyranosyl-2'-hydroxyresveratrol 4'-(6-O-β-D-glucopyanosyl-β-D-glucopyranoside) (2),mulberroside A (3),mulberroside E (4),mulberroside F (5),transoxyresveratrol-4'-O-β-D-glucopyranoside (6),trans-oxyresveratrol 3-O-β-D-glucopyranoside (7),cis-mulberroside A (8),3,4-dimethoxyphenyl β-D-apiofuranosyl-(1→6)-β-D-glucopyranoside (9),kelampayoside A (10).CONCLUSION Compounds 9 and 10 are isolated from genus Morus for the first time,compounds 1,2,4-7 are first isolated from this plant.
